“Qualified” vs. “Licensed” Social Worker in Long Term Care

I want to pass along a recent citation of which you may be interested.  The facility has 2 full time Social Service staff.  One has a Bachelor of Social Work, but is unlicensed.  The other has a Master in Psychology.  Here is the citation:

    “F 251 Qualifications of Social Worker greater than 120 Beds

    ….this requirement is not met a/e/b:  Based on South Carolina Code of Laws, Title 40, Section 40-63-30 a qualified social worker must be licensed to practice social work in the state…..The Administrator was referred to the South Carolina State Laws web page related to social workers must be licensed in the State of South Carolina in order to be identified as a qualified social worker.  The Administrator was also referred to the federal guidelines that indicated some State or local laws are more stringent that the Federal requirement on the same issue”

So, if your facility is greater than 120 beds, you must employ a full time LICENSED Bachelor (or Master) of Social Work.  The stipulation of requiring a year of supervision was not noted; however, it could have been included as well.

I have noticed a lot of searches for “PASRR”.  So I have decided to offer several resources I have found helpful in PASRR compliance.  First, let me say that although PASRRs are a federal requirement, there is not a federally mandated form.  Therefore, states vary greatly in format.

“The current report outlines the findings from the second phase of the study, which involved a national survey of the relevant agencies in all 50 States and the District of Columbia to determine how they have organized and administered Federal requirements under PASRR. The second phase of the study also included case studies of four States, which were selected to include each of the entities that conduct Level II screens: private mental health agencies, community mental health centers, individual mental health practitioners, and referring agencies or State agencies. In each State, a total of six nursing homes were selected, with three located in an urban county and three located in a rural county. In each of those 24 nursing facilities, the administrators were interviewed about the PASRR process, and the medical records for the nursing facility residents were reviewed. In two of the four States, clinical interviews were conducted with a total of approximately 50 nursing facility residents.”

MDS 3.0 Technical Information:
• The “RUG-IV Medicare SNF PPS CMIs 10-01-2010.pdf” document in the RUG-IV package has been found to have labeling errors in the “Hierarchical Classification” column for 4 groups (CD2, CD1, CB2, and CB1). The CMI values are correct and are associated with the correct group. This document has been replaced by a new version (“RUG-IV Medicare SNF PPS CMIs 10-01-2010 V2.pdf”) with the labeling errors corrected.
• A RUG-III Errata document has been posted. In three documents in the RUG-III Version 5.20 grouper package on the “MDS 2.0 Software and Specifications” page, the incorrect ADL ranges were given for groups RAB and RAB for the 34-group model. These are only documentation errors and all code (SAS, C++, or DLL) is correct.

What You Need To Know About Me

The purpose of this notebook is to provide a way for caregivers to communicate with others who provide respite care for the person in your life who has Alzheimer’s disease or other dementia.  This includes family and neighbors as well as non-medical sitter services.  It is also a tool to track behaviors and effective/non-effective approaches to the behaviors.

Creating a Pill Card

A pill card is a simple, visual way to show all the medications a person takes on a regular basis.  The pill card uses pictograms and is especially helpful for those having some difficulty reading/referencing/keeping track of daily medicines.  (AHRQ Publication No. 08-M016; February 2008)

Where to start…..

     I was planning to sleep in today, but as usual my mind started racing.  What in the world would the first resources listed be?????  After getting my cup of coffee and the dogs settled, I stopped to prioritize my morning through prayer and a devotional.  I just love to read My Utmost for His Highest.  One statement stood out to me this am.  “God does not tell you what He is going to do-He reveals to you who He is”.  I am a person who wants a plan, wants answers, structure, etc.  I love it when He puts things in perspective.

     Here we go!!!  Let’s start first with Senior Resources.  (I haven’t decided yet if I will use “senior” or “elderly”.  Any preferences?)  The obvious is:

CMS is a federal agency administering the Medicare program and works in partnership with the States to administer the Medicaid program. 

CON:  This website is overwhelming!  You have to have the patience to find what you are needing.  Even then, the information you find may be generic in nature.  I just returned from checking my link and it was no longer a link.  After 30 minutes of searching, I have finally found what I was originally looking for!

This link will take you to surveyor training.  You will not receive any “credit” for taking courses.  However, this is the training that surveyors receive for various topics.  It is an excellent resource for you.   Click “Archived Webcasts” and it will give you a list of past trainings.  Several titles include:  “MDS 3.0 Part 1-An Introduction”, “Quality Indicator Survey Process”, “Dealing with Turbulence in Organizations”, “Listening, It’s  Not Just Hearing the Words”, and (my favorite), “Mental Illness in Nursing Homes”

This link publishes updates to the federal regulations for hospitals, nursing homes, hospices, etc.  In other words, if you have a hard copy manual or an internet copy, you will need to update your existing with the changes.  I do not receive notifications of changes.  I have to look every so often.
